Where can I go to give samples?

GetCheckedOnline is not available at all LifeLabs locations.

Once you have created a lab form, take it to a participating LifeLabs location. Use the map below to find a participating LifeLabs. At the lab, the staff will explain how to give a urine sample, will take a blood sample and may give you a swab kit to take home.

The LifeLabs website (locations.lifelabs.com) has the most current information about hours of operation and wait times (if walk-ins are accepted).

To make an appointment visit locations.lifelabs.com and search for a GetCheckedOnline location.

The name you use to make your appointment is never attached to your samples or test results, it is only used for the appointment itself. Your samples and results are never attached to your name (only your unique GetCheckedOnline account number).

If you do not want to use your real name to make an appointment you can do the following:

• Online: at step 3 before confirming the appointment, click on the box that says "Enter another person's name". When it asks you to select who the appointment is for, you can use a fake name or initials.
• By phone: call LifeLabs at 1-800-431-7206.
